How to fill out the
Health History Form for your get-acquainted session:

There are two ways to fill out the Health History form:

 

1. Download it here.  Print it out, fill it out, and fax to me at (703) 569-7406.

 OR

If you'd rather fill it out and submit the form on line, please follow these directions:

 

1. Go to my other website (sorry).

2.  Click on the tab at the top that says Contact.

3. Scroll to the bottom of that page where you'll see, "If you have already scheduled a consultation, please click here to fill out the Health History form." 

4. Fill out the form there.  It will take about 10 minutes.

5. Click Submit, and I'll have it ready for our appointment.

Is it really possible to get healthier once you have high blood pressure, high cholesterol, or type II diabetes?

Yes, absolutely.  Those conditions are a result of choices over time.  Start making different choices NOW and your health FUTURE can be very different from your health HISTORY.

There is hope!  I used to be in the prediabetic category.  I have Type II Diabetes all through my family, and my grandmother suffered many mini-strokes, high blood pressure, cataracts, macular degeneration, and peripheral neuropathy before she finally died of cardiovascular disease.  My mother may have had diabetes, but she died of colorectal cancer before it could be confirmed. 

I decided very early, when my mother died at the age of 41, that I was going to have a different health history.  I started making different choices, evaluating a number of foods and tools, and now I can teach you!  Get the full story here.
